We participate in the GP Chronic Condition Management Plan (GPCCMP), previously called a Chronic Disease management (CDM) or Extended Primary care (EPC) plan. This Medicare supported, referral service generally provide a maximum of 5 chiropractic visits per calendar year and need to be initiated by your GP as a referral to our clinic, Bayside Health Professionals. As at 1/11/25, the rebate is $61.80 for each of the 5 eligible visits.
